For home health

Clients spread across the city, and you still know who needs a visit first

Between visits, AdCare keeps watching. Coordinators see which clients broke from their routine, and caregivers arrive with context instead of starting cold.

AdCare supports coordination and handoff. It does not replace visits and it is not a medical device.

WHAT YOU’RE ASKING

The four questions agencies ask first

How many clients can one coordinator follow?

What changes is that coordinators stop calling around to find out who is fine — the list sorts itself by who needs attention, so the hours go to people who actually need them. The number itself has to be measured on your own client mix; we pilot and compare rather than promise up front.

What app do caregivers use?

An app on their phone: the visit schedule, context on the client before walking in, and a record of what was done on the way out. That record replaces the end-of-shift paperwork.

What about clients far out with weak internet?

Readings keep being taken and stored on the equipment in the home, and recorded reminders keep running, so a weak line does not create a gap in the record — data queues and sends later rather than being lost. What does need the connection is the assistant answering new questions and live alerts reaching the coordinator.

How are shift handoffs recorded?

Every alert, visit, and caregiver note is time-stamped. The next shift opens it and sees what was handled, instead of depending on a text message between two people.

ROLLOUT

Three steps per client

  1. STEP 01

    Install during a normal visit

A caregiver sets up the screen and pairs the ring during a regular visit. No separate appointment, no drilling.

  2. STEP 02

    The system learns each client

Every home has its own rhythm. Learning per client keeps alerts tied to the actual person instead of one threshold for everyone.

  3. STEP 03

    Coordinate by who needs attention

This morning’s list sorts itself, so extra visits go to the people who actually need them.

WHO USES WHAT

Three roles, three different screens

Coordinator

A list sorted by attention needed

Who broke routine, who has gone longest without a visit, whose alert is still open.

Caregiver

The field app

Context before walking in, the tasks for this visit, and a record on the way out.

Client’s family

Alerts and daily activity

Families far away worry less, and the agency fields fewer check-in calls.

Management

Exportable logs

Alerts, visits, response times — records for documentation and for reporting.

WHAT THE AGENCY GETS

What’s included

  • Coordination board

Every client on one screen, sorted by who needs attention.

  • Caregiver app

Schedule, context, and on-site records. Runs on their own phone.

  • In-home equipment kit

The AdCare screen, a smart ring, and optional monitors depending on the client.

  • Handoff log

Written from real actions, exportable per client and per date range.

  • Family portal

Enabled per client. Families see what the agency chooses to share.

  • Client and caregiver, each in their own language

The client’s screen and the caregiver’s app are set separately, so neither has to work in a second language.

Frequently asked questions

Does this replace visits?

No, and we do not sell it that way. AdCare covers the gap between visits and helps you order the day. Someone still has to show up.

Does it connect to our agency management software?

Phase one runs alongside it — nobody is asked to drop what they have or relearn a workflow mid-stream. How far your platform connects, we go through in the demo.

What happens to the equipment when a client leaves?

Because nothing is drilled or wired, the kit comes off and goes to the next client. Wiping the old data and reconfiguring it is our job, not your team’s.

Is it priced per client or per agency?

Priced per active client, with tiers by volume. We cover the structure in the demo, including the minimum to run a pilot.
